Congratulations to the Salford Primary Care Together and NHS Salford Clinical Commissioning Group team who recently graduated from Haelo’s Improvement Science for Leaders programme.
The team has successfully completed Haelo’s year-long programme. They chose to focus on the two week wait referral process within the Salford Wide Extended Access Pilot. Through the quality improvement methods they learned during their study, the team have been able to adapt and improve the process, creating a safer and more reliable way of handling two week wait referrals for patients.
Well done to the team: Laura Hosey-Davies, Primary and Community Informatics Manager at NHS Salford CCG and Salford Primary Care Together’s Chris Nortcliff, Head of Clinical Services, Moira Ogunsakin, Lead Practice Nurse, Diane Reece, SWEAP Service Co-ordinator and Clare Fozard, GP Fellow in Quality Improvement.
